Supporting Students with Prevalent Medical Conditions

To ensure your child’s safety at school, families and school staff work together to create an up‑to‑date Plan of Care for students with prevalent medical conditions such as asthma, anaphylaxis, or epilepsy.

What Families Can Do

  • Notify the school — Let the school know if your child has a prevalent medical condition, and inform them of any changes throughout the year.

  • Meet with school administration — Share important information about your child’s condition so the school can develop an effective Plan of Care.

  • Submit the yearly consent form — Provide the school with the annual Consent to Administer Medication form signed by your family doctor. The form is available on the UCDSB website.

  • Provide required medication — Send all necessary medication (e.g., EpiPen, inhaler) to the school at the start of the year, along with the signed consent form.

  • Teach your child about their condition — Help your child understand their triggers, symptoms, and when to follow their Plan of Care.

Annual Review

A meeting will be held early each school year to create, review, and update your child’s Plan of Care to ensure it reflects their current needs.
